From the Throne of Destiny Expansion.

In the Throne of Destiny expansion, players have the choice of aligning themselves with one of two factions. Each has its own series of quests and a player aligned with one may not complete the quests of the other faction. The quest for the Dericost Tome in the Dericost Ruin is the first in the Sanamar series.

To begin, speak to the Grand Knight in Sanamar. He can be found at the Great Hall on the hill behind the city.

NPC: Grand Knight - 70.0N, 62.5W (Sanamar)

Grand Knight tells you, "You're new to Sanamar, are you not? If you desire to prove your loyalty to the King, I have a task you may find interesting."
Grand Knight tells you, "The King has been informed of an ancient tome which lies in a ruined underground city. He dispatched a company of knights to the ruins, but they were repelled by the beasts within. The craven dogs spread tales of ghosts and undead before I had them executed for cowardice."
Grand Knight tells you, "I need someone to enter the ruins and retrieve this tome. If you do this for me, I may have more tasks for you in the future. Of course, if you fail or otherwise betray me, then I'll have nothing to do with you ever again."
Grand Knight tells you, "You can find the ruins near the outpost of Westwatch at 74.2N 74.2W. I expect to hear from you soon."
Grand Knight tells you, "But know this - the Grand Mother of Silyun also seeks this tome. Complete this task for me and she will have nothing more to do with you. Though, why you would want to socialize with those damnable rebels is beyond me."

To reach the Ruin take the Westwatch Outpost portal at 71.2N, 61.3W. The dungeon is  a short run away.

Dericost Ruin - 74.2N, 74.2W

From the drop, head past the first gated door to the passage leading north into a large room. Exit the room east and keep to the right down a series of ramps. Next take the ramp up to the south and keep to the left. You'll come to a room with the book, Ancient Discoveries, spawning on the ground.

Exit the room the way you entered and at the first 4 way go north, and keep to the left until you see the Dericost Ruin key spawning on the floor.

Exit the key room north, then keep to the right until you first go up a ramp, then down. Head north to the wall, then you'll see the locked door to the west. Use the key on the door and you'll find the Ancient Tome on the ground near the surface portal.

Timer: 7 days

Return to Sanamar and give the Grand Knight your finds for your rewards!

You give Grand Knight Ancient Discoveries.
Grand Knight tells you, "Ah, Sir Binwas. A finer warrior have I seldom met. He deserves not the humiliation he suffered at the hands of that wench."
Grand Knight grimaces with hatred.
Grand Knight tells you, "Eleonora deserved everything we did to her."
You've earned 5,000 experience.
Grand Knight gives you Kam'shir.

You give Grand Knight Dericost Tome.
Grand Knight gives you Friend of Sanamar Token.

Grand Knight tells you, "You have done well. Feel free to stay in Sanamar as long as you like."
You've earned 20,000 experience.

You give Grand Knight Friend of Sanamar Token.
Grand Knight tells you, "May your loyalty to the glory of Sanamar be apparent to all you meet."

Speak to the Grand Knight again and he'll start you on the 2nd quest of the Sanamar series, the Knights of Karlun.
